Doesn't the second half of the Spanish definition (from DRAE!) refer to hermanastro, not hermano ?

blood brother in en-GB is a synonym for brother, but is usually used only to emphasise the status of the relationship in the presence of half- and or step-brothers, as in "Jack's only my step-brother. Joe is my blood-brother - he's for real!"<br />
In the US, there's the Amerindian concept, which is something quite different...

The Drae definition also surprised me when I was copying it, but it could make sense: my half brother shares the same blood with me (either from my mother or my father, while a step brother could also include the children of my stepmother/stepfather from a previous marriage... (am I right?)

Regarding the other concept for "blood brother", it seems to me also that "hermano de sangre" would be the concept of someone that becomes your brother through a "blood pact" (be it literal or figurative: a friend who is more than a brother to you...) (or am I introducing a false cognate?)
